I-20 Request form

This form is only for current students and students on active OPT who need a reprinted I-20 for travel, DMV, or SSN.

Allow 3-5 business days for processing. The new I-20 document will be signed electronically and will be emailed directly to your �첥��Ƶ student email ONLY. You should print the document and sign page 1. Services such as the DMV and SSN administration should accept electronically signed I-20s.

Verification Letter Request Form
Use this form to request for a letter that verifies you are an international student at �첥��Ƶ.

We can verify the following information:
- Your major
- The dates of your attendance at �첥��Ƶ
- Number of units enrolled in each semester
- Fees paid or unpaid
- Dates on your I-20
We cannot verify your grades or academic standing. You must order official transcripts for proof of your grades and academic standing.
Medically required drop below 12 units at �첥��Ƶ (Medical RCL/CE)

If you are not able to maintain and complete 12 units in Fall or Spring semester you must be authorized for Reduced Course Load OR Concurrent Enrollment.

If you specifically require a medical RCL, you may have your doctor (M.D., D.O. or clinical psychologist) in the U.S. complete and directly submit the Medical RCL letter template

RCL approval for any reason is required BEFORE dropping under 12 units.
Transfer Out Request Form
Accepted to another institution? Congratulations! You must have your SEVIS record transferred to new institution. Follow these steps to request for the SEVIS transfer:
1. Complete the and upload a copy of your acceptance letter.
2. Your Transfer must be completed within your 60 day grace period for transfer- counted from the last day you attended classes at �첥��Ƶ. If you miss the grace period, �첥��Ƶ will be unable to transfer your SEVIS record in active status!
3. U.S. Immigration regulations require that you must attend the next available session at your new school.
4. If you plan to travel outside of the U.S. before classes begin at your new school, you will need to use the new school's I-20 to re-enter the U.S.
5. If you are currently on OPT from �첥��Ƶ, you will also need to submit a copy of your OPT card and a letter of verification of employment from your employer to mhalley@occ.cccd.edu. Your OPT will end when we transfer your SEVIS record.

WAVIER: ONLY for use by government sponsored students (i.e. SACM or UAE), students covered under a parent/spouse employed by a U.S. employer, or students on active OPT insured through an employer plan.

REFUND: Eligibility is subject to review; only available up to 45 days from coverage start or date of purchase, whichever is first. Student who have coverage as described above (waiver) or who have requested Termination - Authorized early withdrawal and have returned or will be returning to their home country immediately.

Read the instructions on the form carefully to determine if you are eligible for a refund or a waiver.

This form is to be used exclusively by International Students only. If you are not an international student, please visit the �첥��Ƶ Enrollment Center -FERPA (Family Educational Rights & Privacy Act) for more information.

International Students:

Use this form to authorize a third party (agent, parents, relative, or friend) to access your student information. In order to maintain student confidentiality, your �첥��Ƶ student information is not accessible to anyone other than you, unless you fill out this form. For example, if you want to have someone pick up your I-20, you will need to complete this form with their information. The authorized party will only be able to obtain your personal information in person (not over the phone) and with a valid photo ID. We must receive the form directly from you.
